Yeah, I noticed that too. It seems that D only calls Leon by his first name during moments of emotional intimacy and/or when Leon lets his guard down. And if I recall correctly, D also refers to Leon by his first name when D gets all excited over the Nazca line drawings in the El Dorado story.
